AOM celebrates the successes of Primary Health Care Collaboration

As Ontario moves towards some significant changes in structuring of primary health care through formation of health teams, the AOM revisits a success story that shines light on how to realize this monumental task successfully.
With a system that manages to provide excellent services to clients through a community-centred partnership, Midwives Clinic of East York-Don Mills and Michael Garron Hospital set the tone on how health teams could have space for all primary care givers to collaborate effectively.
